A man died in a freak incident after falling into a compactor at a Texas-based UPS facility, according to officials.

Units with the Dallas Fire Rescue responded to an "industrial accident" report at a UPS location in Dallas just before 11 a.m. May 16. Officials said the individual "fell into the trash compactor," ultimately leading to his death.

"As part of that response, the Urban Search and Rescue (USAR) Team was dispatched to extricate the person, whose state was unknown at the time," a Dallas Fire-Rescue spokesperson told USA TODAY in a statement. "However, it has since been confirmed that the person inside the compactor is deceased."

According to 11 Alive, the Dallas County Medical Examiner identified the victim as 68-year-old Juan Pablo Chavez Palacios. The Occupational Safety and Health Administration is reportedly conducting an investigation into the incident. Additional details were not immediately made public.
